POTTSTOWN PA – Two new family medical providers have arrived at Community Health & Dental Care (CHDC), 351 W. Schuylkill Rd. Certified nurse practitioners Marqueta N. Smith, MSN, APRN, FNP-C (above left), and Jennifer Heffernan, CRNP (above right) have joined its staff.
Heffernan “comes to CHDC with extensive experience as a licensed nurse practitioner and is happy to be back serving the Pottstown community,” the organization reported. She previously worked at Collegeville Internal Medicine, and earlier was a dedicated family nurse practitioner at Brookside Family Practice for nearly 10 years.
Heffernan earned her master of science degree in nursing from Thomas Jefferson University in Philadelphia, and her bachelor’s in nursing from York College, York PA.
Smith “is a dedicated, licensed family nurse practitioner with (more than) 15 years of combined experience as an advanced practice provider and telemetry nurse, CHDC said. She “is committed to providing exceptional patient-centered care, and serving at-risk populations using evidence-based practice to improve outcomes,” it added.
Smith earned her master of science degree in nursing from Widener University in Chester PA, and her bachelor’s in nursing from Gwynedd-Mercy College, Gwynedd Valley PA.
Appointments with both are available now for prospective patients ages 18 and older. To schedule appointments, call CHDC at 610-326-9460.
About CHDC
Operating since 2008, Community Health & Dental Care said it intends to “ensure access to appropriate levels of care for all people in (its) service area regardless of their ability to pay.” Same day and integrated appointments are available to meet the scheduling needs of patients and family members.
It provides medical, dental, OBGYN, vision, podiatry, physical therapy, medication-assisted treatment (MAT), integrated behavioral health, care and case management, and free transportation to and from all its locations in Pottstown and Barto. In July, the center said it expected to open a new medical office in Boyertown offering pediatrics and women’s health.
