Introducing Community Health & Dental Care of Pottstown

POTTSTOWN PA – Community Health & Dental Care Inc. (CHDC), founded in Pottstown during 2008, operates across greater borough-area communities as a designated federally qualified health center. Thanks to its government funding, it is able to offer significant discounts on a variety of services for qualified patients.

Its largest offices provide the widest range of services. They are located minutes from Pottstown in North Coventry Township, Chester County, at The Shoppes at Coventry shopping center, 351 W. Schuylkill Road Suite G15-A.

As of January 2024, Community Health & Dental Care also offers:

  • Medical services in Lower Pottsgrove Township, Montgomery County, at 700 Heritage Dr. Suite 701;
  • Dental services in Lower Pottsgrove, 800 Heritage Dr. Suite 802; and
  • Medical, dental, and physical therapy services in Barto, Berks County, at 1315 Route 100 North.

Office days and times vary by location. CHDC provides free transportation to patients who request it to-and-from specific stops in Pottstown, Spring City, Boyertown, Pennsburg, Hereford, and Birdsboro PA on Mondays through Fridays from 9 a.m. to 3 p.m. Transportation hours may be subject to occasional change.

The organization said it “provides medical, dental, OBGYN, vision, podiatry, physical therapy, medication-assisted treatment (MAT), integrated behavioral health, care and case management.” It intends to ensure “access to appropriate levels of care for all people in the service area regardless of their ability to pay.”

Same day and integrated appointments are available to meet the scheduling needs of patients and family members, it added.

Community Health & Dental Care said it accepts as payment “most major commercial, Medicare and Medicare supplement plans, fee-for-service Medicaid and managed care Medicaid plans,” and provides “a health care discount for eligible uninsured and under-insured patients.

Under its 2012 partnership with the federal Health Resources and Services Administration, qualified patients can receive an up to 100-percent healthcare discount. Its nominal fee reported during January 2024 for a medical, behavioral health, or vision visit is $20; dental visit, $35; and prescription, $5.
